Greater Metropolitan Youth Football League Identification (GMYFL ID)

Each player must obtain a State Issued Identification Card. The card is accepted as official player identification at all GMYFL AAU certifications, all GMYFL AAU sponsored games and events, and upon demand by the GMYFL AAU League officials or Board of Directors, pursuant to the Boards official duties.

Accepted Player Identification:

•    STATE MOTOR VEHICLE ADMINISTRATION IDENTIFICATION CARDS

State issued Motor Vehicle Administration ID cards may be obtained from all local (DMV) offices for minimal charge and are good for 5 years. You must provide Birth certificate and Social Security Card and Parents’ Identification.

      •     OFFICIAL MILITARY IDENTIFICATION CARD (Active Duty)

Identification cards issued by various branches of the Armed Forces of the United States of America, Cards must be current. Card must display photograph, date of birth, date of issue, expiration date, players' printed first and last names, and signature of parents for those players less than ten years of age.

      •     UNITED STATES PASSPORT

As issued by the United States of America, Department of State. Card must be current. Card must display photograph, date of birth, date of issue, players' printed first and last names, and signature of parents for those players less than ten years of age.

      •     MARYLAND NATIONAL PARK AND PLANNING RECREATION CARD

The MNPPC REC CARD is acceptable identification however cards must display photograph, date of birth, date of issue, expiration date, players' printed first and last names, and signature of parents for those players less than ten years of age.

•    GMYFL LEAGUE ID CARD

The parent must bring the player to the designated club certification location on the designated date to obtain a league issued ID.  The following forms are accepted for D.C. residents and special cases only

1. Original Birth Certificate or (CERTIFIED COPY WITH STATE SEAL) and a plain Xerox copy (to be left with the Club)

2. Passport and one plain Xerox copy

3. Resident Card issued by US Government and one plain Xerox copy

In addition to the above forms; D.C. players will need to verify proof residence by providing (one of the following)

· Utility bill; telephone, cable TV bill

· Checking or Savings Acct Statement

· Lease

· Credit Card Bill

· D.C. Tax Return

· Property Tax Bill

· Cancelled Check.

· Report card

· Health form for school

· Immunization form -- -----
